How Forme works,
and what to expect.

These terms govern your use of the Forme Health platform and services. By creating an account or placing an order, you agree to these terms. Please read them — they're written to be clear, not to bury anything important.

The service

What Forme is — and what it isn't.

Forme Health Limited operates a telehealth platform that connects patients with New Zealand-registered doctors for the purpose of assessing and treating male pattern hair loss. We provide the technology, the clinical pathway, and the subscription service that delivers treatment to your door.

Forme is not
  • A general practice (GP) service — we treat hair loss only
  • A diagnostic service for conditions outside of male pattern hair loss
  • An emergency service — if you are in urgent need of medical attention, call 111

Forme Health Limited is not a healthcare provider. Clinical decisions are made by independent New Zealand-registered medical practitioners. Forme facilitates access to those practitioners — it does not direct or override their clinical judgement.

The prescribing model

How your prescription works.

When you complete an online assessment, your responses are reviewed by a New Zealand-registered doctor. That doctor independently determines whether treatment is appropriate for you and, if so, what treatment to prescribe. This is a genuine medical assessment — it is not automatic.

If the doctor determines that a prescription is appropriate, they will issue a prescription that is dispensed by a New Zealand-registered pharmacy and sent directly to you. You have the right to ask questions about your prescription, request changes, or decline treatment at any point.

Prescription medicines carry risks as well as benefits. Your prescribing doctor will consider your individual health history before prescribing. It is important that you provide accurate and complete information in your assessment. Providing false information may result in treatment being prescribed that is not appropriate for you, and Forme accepts no liability for outcomes arising from incomplete or inaccurate patient-provided information.

Medical emergency: Forme is not equipped to handle medical emergencies. If you experience a serious adverse reaction to any medicine, stop taking it immediately and seek urgent medical attention by calling 111 or going to your nearest emergency department.

Subscription terms

Billing, renewal, and cancellation.

How billing works

Forme operates on a monthly subscription basis. Payment is taken at the time of your first order and on the same date each subsequent month. Your treatment is dispensed and shipped following each successful payment.

Automatic renewal

Your subscription renews automatically each month until you cancel or pause it. You will receive a reminder before each renewal. By maintaining an active subscription, you authorise Forme to charge your payment method on each renewal date.

Pausing your subscription

You can pause your subscription at any time through your account dashboard. When paused, no further payments will be taken and no further shipments will be sent. Your subscription will remain paused until you choose to resume it. Pausing does not delete your account or medical records.

Cancellation

You can cancel your subscription at any time through your account dashboard. Cancellation takes effect at the end of your current billing period — you will not be charged again, and your final shipment will be the one already dispatched or scheduled for that period. We do not offer refunds for partial billing periods that have already been charged.

Price changes

If we change our subscription price, we will notify you at least 30 days in advance. You will have the opportunity to cancel before the new price takes effect.

Your responsibilities

What we need from you.

For Forme to operate safely and for your doctor to make appropriate prescribing decisions, you agree to the following obligations as a patient.

Accurate and complete disclosure

You must provide accurate, honest, and complete information in your assessment and at all times during your subscription. This includes your full medical history, current and recent medications, known allergies, and any conditions that may affect your suitability for treatment. Omitting or misrepresenting information may result in treatment that is inappropriate for you — and Forme accepts no liability for outcomes arising from incomplete or inaccurate patient-provided information.

Reporting side effects

If you experience any side effects or adverse reactions during your treatment, you must notify your Forme doctor promptly through the platform. For severe reactions, stop taking your medicine immediately and seek urgent medical attention. Do not wait for a Forme response in an emergency — call 111.

Updating your health information

If your health circumstances change materially during your subscription — including new medical diagnoses, new prescription medicines, or planned surgery — you must inform your Forme doctor before continuing treatment. Your doctor may need to review your prescription in light of new information.

Personal use only

Prescription medicines dispensed through Forme are prescribed specifically for you based on your individual health assessment. You must not share your prescribed medicines with any other person. Sharing prescription medicines is illegal in New Zealand and may cause serious harm.

Age requirement

You must be 18 years of age or older to use Forme. By creating an account, you confirm that you meet this requirement.

Liability

Limitations on our liability.

Forme provides a platform that facilitates access to independent medical practitioners and pharmacy services. Clinical decisions — including whether to prescribe, what to prescribe, and at what dose — are made by those independent practitioners, not by Forme.

To the maximum extent permitted by New Zealand law, Forme's liability for any claim arising from your use of the service is limited to the total subscription fees you have paid in the 12 months immediately preceding the claim.

Nothing in these terms limits your rights as a consumer under the Consumer Guarantees Act 1993 or the Fair Trading Act 1986, or limits Forme's liability for death or personal injury caused by our negligence.

We are not liable for delays or failures caused by circumstances outside our reasonable control, including courier delays, pharmacy fulfilment issues, or regulatory changes affecting the availability of medicines.

Governing law

New Zealand law applies.

These terms are governed by the laws of New Zealand. Any dispute arising from or relating to your use of the Forme service will be subject to the exclusive jurisdiction of the courts of New Zealand.

Forme Health Limited is incorporated in New Zealand. Our services are designed and operated in accordance with New Zealand law, including the Health Practitioners Competence Assurance Act 2003, the Medicines Act 1981, the Privacy Act 2020, the Consumer Guarantees Act 1993, and the Health and Disability Commissioner Act 1994.

Forme Health Limited reserves the right to update these terms from time to time. Where changes are material, we will notify you by email at least 14 days before they take effect. Continued use of the service after that date constitutes acceptance of the updated terms.
